Inquiry form
Complete inquiry form for appointment. Please contact us to confirm your inquiry With RDU Assessments & Advocacy.
Intake Form
Complete intake documents. Intake forms will be sent via email after completing inquiry form.
testing consultation
Schedule virtual testing consultation session with psychologist – 1 to 1.5 hours
testing session
Schedule & complete testing session(s) with psychologist – 1 to 3 hours per session
Complete Report
Receive completed report including a diagnosis (if applicable) and specific recommendations – 2 to 3 weeks after the last testing session or after the last document related to testing is received
Review Session
Complete review session with psychologist to review report, diagnosis, and recommendations as well as to provide an opportunity for questions regarding the report – 1 to 1.5 hours
Superbill
After the review session, a superbill will be provided to file an out-of-network claim with your insurance provider.
Disclaimer
It is recommended that your insurance provider be contacted in advanced to determine what your insurance plan covers regarding out-of-network providers. The administrative team can provide some support regarding this process; however, it is ultimately the responsibility of the client/client’s caretakers to be informed regarding specific health insurance coverage.
